Advance Care Planning (ACP)

“Mr Tan, we are able to stabilise your wife’s condition but she’s not out of the woods yet. However, the medical episode had impacted her ability to communicate. We would need to discuss with you (and family) about her treatment options, including the consideration for a second surgery. The outcome from the surgery would be uncertain… Do you know Mrs Tan’s preferences when it comes to her care and treatment?” 

More often than not, individual’s preferences on their care arrangements and treatment options are not known to their loved ones. When accident or serious illness occurs, it is a burden for loved ones to make decisions on behalf of the patient.

To avoid Mr Tan’s situation on having to burden your family with your medical decisions, you can consider doing your own Advance Care Planning (ACP) and appoint loved one(s) to be your Nominated Healthcare Spokesperson(s) (NHS).

Understanding what is ACP

ACP focus on enabling NHS(s) and loved ones to be aware of your preferences and wishes, in relation to treatment and care arrangements, should you no longer be able to make decisions for yourself.

To do so, one or more sessions can be arranged, involving you, loved ones, and a Montfort Care ACP facilitator.

During the session, the coverage would include:

Exploring your values, beliefs, and motivations, and preferences on treatment and care arrangements should you loses your mental capacity.Engaging you and loved ones to ensure that your stated preferences can be understood by loved ones and the medical team.Appointment of your designated NHS(s) and explaining on the role of the NHS.


Who is suitable for ACP?

ACP is suitable for anyone who is 21 years old and above, with decision making capacity.  However, we would encourage the following groups to consider ACP:

• Healthy residents who are age 60 and above

• Individual diagnosed with early chronic condition but remains independent

• Patient whose cognition is deteriorating but retains decision making capacity

Loved ones who are above 21 years old can be appointed to be your Nominated Healthcare Spokesperson (NHS). NHS will be your voice to share your preferences with the medical team. Up to two loved ones can be appointed to be your NHS.

Starting your ACP journey

You can start to make plan on your ACP through 5 steps:

1.      Understand about ACP

·        You may also explore the ACP workbook which allows you to start thinking about your values, wishes, and preferences. Also available in Chinese, Malay, and Tamil.

2.      Think about your preferences

·        What is important for you to live well, your personal values and wishes?

·        What are some of your personal lifestyle you would wish to maintain despite a loss of mental capacity?

3.      Decide on your preferences and NHS(s)

·        Have you decided on your preferences and designated NHS(s)?

4.      Discuss with your loved ones

·        Have you shared with loved ones about ACP, and discussed with them about your preferences and the choice of NHS(s)?

5.      Doing your ACP

·        Have you and your NHS(s) approach an ACP facilitator to have an ACP discussion to have your preferences documented down?
