Mental Health Assessments

Our Programmes

Community Intervention Team (COMIT)
COMIT offers holistic support for mental health and dementia clients and caregivers. With counsellors, therapists, nurses, and coordinators, it provides assessments, counselling, and personalised care plans. This community-based approach also equips caregivers with coping skills to overcome challenges and provides in-depth information about mental health conditions.
Community Outreach Team (CREST)
CREST serves as a community network for people with dementia and depression, as well as caregivers who need the additional support to care for their loved ones. CREST links the community with the appropriate mental health care and social support networks through outreach and education.
Post-Diagnostic Support Community Outreach Team (CREST-PDS)
The CREST-PDS programme offers structured support for persons newly diagnosed with dementia and their caregivers. It provides information, links to services, and case management for six months upon referral. A team of community liaison coordinators and an allied healthcare professional help them understand their condition, access community resources to support person-centred care, and facilitate early disease management.
